Birth Certificates

Whenever a baby is born in a hospital, the hospital creates a certificate of live birth for that child and sends it to us. Parents of babies born at home must come to us (or go to a hospital) to register the birth and obtain the Birth Certificate. A notarized third-party Statement of verification of the occurrence of a live birth is required. A witness fills out this form, verifying that the child was born to the woman who claims to be the child’s mother.

How to obtain a copy of a Birth Certificate

Parents who are named on the Birth Certificate or legal guardians may call or write us for a copy of the child’s Birth Certificate. Come to the Health District Main Building at 201 W. Kenyon Rd., Champaign, and visit the Registrar at the front office. You must bring a photo ID (preferably a driver’s license) or court papers verifying that you are a legal guardian of the child. You will be asked to complete an Application for Birth Record. Birth Certificates cost $10 for the first copy, $2 for each additional copy per child. You may also write to request a Birth Certificate. You must provide a photocopy of a photo ID of a parent named on the Birth Certificate, or proof of legal guardianship. Fill out the Application for Birth Record and mail it in with your request.

All written requests will be processed within one business day of their receipt.

NOTE: The Health District keeps birth certificates for those born in Champaign County in the past 10 years. For birth certificates of those born in Champaign County more than 10 years ago, please contact the Champaign County Clerk's office.
