adult health promotionNurses visit adults at home to help monitor and maintain their health. The nurses also offer education and referrals to other services as needed. These services are offered throughout Champaign County.

Eligibility Any adult living in Champaign County who needs nursing case management. (Most of our clients have one or more chronic medical conditions.)
Cost These services are FREE to eligible adults.
How do I get services? We accept referrals from physicians, family members or social service providers.
